| Fall Academy Registration |
To register for an academy program, including the Fire, Chargers, Predators, or Inferno team, please do the following:
1. Complete the online registration form below
2. Mail the following items to Amy Sokol, 3136 Ben Davis Avenue, Lower Burrell PA 15068:
1. Completed US Club Registration form
2. Completed PFA Indemnification form
3. Copy of your child's birth certificate
4. One inch by one inch photo of your child
5. Check payable to PFA in the appropriate amount.
3. If your child will be playing on the Fire, Chargers, Predators, or Inferno team, you will also need to go to the PFA Store tab and order a uniform!

| Fall Senior Club Registration |
To register for the fall season, please do the following:
1. Complete the online registration form below
2. Mail the following items to Amy Sokol, 3136 Ben Davis Avenue, Lower Burrell PA 15068:
1. Completed US Club Registration form
2. Completed PFA Indemnification form
3. Completed PaWest Classic League Registration form
4. Two copies of your child's birth certificate
5. Two one inch by one inch photos of your child
6. Deposit check of $250 payable to PFA
3. All players (other than returning Fury players) will also need to go to the PFA Store tab and order a uniform!
